Community Health Needs Assessment and Community Service Plan

Every three years, MSK conducts a Community Health Needs Assessment (CHNA). This is a comprehensive process required by the Affordable Care Act that helps hospitals identify and prioritize the health needs of the communities they serve.

The CHNA process includes gathering and analyzing data on community health indicators, engaging with community stakeholders, and reviewing existing resources to identify key areas for improvement.

Based on the findings of the CHNA, we then develop a Community Service Plan (CSP). The CSP is the strategic plan that responds to the CHNA findings. It outlines specific goals, programs, and partnerships designed to address the CHNA-identified health priorities over a three-year period.

We are conducting a CHNA in 2025, and our CSP will include activities, interventions, and goals for the years 2026 and 2027.
