The clinical program at Memorial Sloan Kettering Cancer Center (MSK) began in 2007 with the pharmacy department’s recognition for the need of advanced clinical training in pharmacy practice. The program has successfully expanded from two adult clinical specialists to more than 30 specialists covering multiple adult and pediatric services within leukemia, bone marrow transplant, lymphoma, multiple myeloma, thoracic, neuro-oncology and phase I services. Clinical pharmacy specialists also cover additional services including infectious disease, geriatrics and pain and palliative care.
Our clinical pharmacy specialists serve on multidisciplinary teams as experts in the therapeutic use of medications and act as liaisons between the department of pharmacy and physicians, nurses, and other disciplines. Additionally, they are actively involved in clinical research, guideline development, and contribute to national organizations essential to their fields.
With the breadth of experience from within our department, our clinical preceptors offer residents direction, leadership and support throughout the residency year with clinical and research responsibilities. Our preceptors are dedicated to the success of each resident and ensuring that the residents training and education will prepare them for clinical and administrative opportunities upon program completion.

Jennifer Thackray, PharmD, BCPS, BCOP, BCPPS
Manager, Pediatric Clinical Pharmacy Services
Director, PGY2 Oncology (Pediatrics) Pharmacy Residency Program
Dr. Jennifer Thackray is the Manager of Pediatric Pharmacy Clinical Services and the Program Director of a PGY2 Pediatric Oncology Residency at Memorial Sloan Kettering Cancer Center Kids (MSK Kids) in New York, NY. Dr. Thackray earned her Doctor of Pharmacy degree from the University of Oklahoma College of Pharmacy and went on to complete an ASHP-accredited PGY1 Pharmacy Practice Residency (2010) and a PGY2 Pediatric Pharmacotherapy Specialty Residency (2011) at the University of North Carolina Health Clinics in Chapel Hill, NC. Dr. Thackray is board-certified in oncology and pediatric pharmacotherapy. Dr. Thackray joined the MSK Department of Pharmacy in 2011, initially practicing in the pediatric oncology inpatient setting. Over her tenure at MSK, Dr. Thackray has assisted in developing pediatric clinical services in the outpatient hematologic malignancies clinic, precepted and mentored residents, and served as a research advisor for PGY2 resident research projects. Dr. Thackray’s clinical interests and research efforts encompass pediatric oncology, supportive care measures (e.g., chemotherapy-induced nausea and vomiting, venous thromboembolism, menstrual suppression), pharmacogenomics, and the development of clinical guidelines within international organizations. She is a member of the Board of Directors for The Pediatric Pharmacy Association (PPA), the Children’s Oncology Group (COG) Pharmacy Steering Committee, and the MSK Medical Staff Advisory Council. She is passionate about increasing and improving interdisciplinary efforts to optimize pediatric oncology care across the healthcare system. Dr. Thackray received the 2021 Ralph D. Bienfang Outstanding Practitioner Award from the Alumni Affairs Board of the University of Oklahoma College of Pharmacy. Dr. Thackray is an active member of the American Society of Health-System Pharmacists (ASHP), American College of Clinical Pharmacy (ACCP), COG, Clinical Pharmacogenetics Implementation Consortium (CPIC), Hematology/Oncology Pharmacy Association (HOPA), and PPA.
Lisa Modelevsky, PharmD, BCOP

Director, PGY2 Adult Oncology Residency Program Director
Dr. Lisa Modelevsky is the Residency Program Director for the PGY2 Adult Oncology Pharmacy Residency at Memorial Sloan Kettering Cancer Center (MSK). Originally from the Chicago area, Dr. Modelevsky earned her Doctor of Pharmacy degree in 2012 from Midwestern University - Chicago College of Pharmacy. She completed her PGY1 Pharmacy Residency with the North Shore LIJ health system in New York, followed by a PGY2 Oncology Pharmacy Residency at Boston Medical Center. Dr. Modelevsky began her career at MSK in 2014 as a Clinical Pharmacy Specialist with the neuro-oncology service, where she served as a clinical preceptor for both inpatient and outpatient neuro-oncology rotations. She later transitioned into leadership roles, including Clinical Pharmacy Manager for multiple services—most recently Lymphoma, Multiple Myeloma, and Neuro-Oncology, and assumed the role as PGY2 Adult Oncology Residency Program Director. Additionally, Dr. Modelevsky serves as the clinical preceptor for the breast medicine outpatient rotation. She is board certified in oncology pharmacy (BCOP) and is a contributing author for BCOP education. Dr. Modelevsky has extensive clinical research experience primarily focusing on optimizing treatment for patients with central nervous system lymphomas and high-grade gliomas. She is an active member and frequent presenter for several professional pharmacy organizations including Hematology/Oncology Pharmacy Association (HOPA) and the New York Society of Health-System Pharmacists (NYSCHP).
Assistant Director, Drug Policy Management, Quality & Safety
PGY2 Medication-Use Safety and Policy Residency Program Director
Amelia Chan is the Residency Program Director (RPD) for the PGY2 Medication-Use Safety & Policy Program at Memorial Sloan Kettering Cancer Center. She has over 25 years of experience in health system pharmacy serving in various positions, including clinical and leadership roles in advancing pharmacy practice. She manages Drug Policy, Pharmacy & Therapeutics, and Quality & Safety for the Division of Pharmacy Services. She is responsible for drug policy development and administration, formulary management, institutional guidelines development, medication use evaluation, and quality and safety improvement.
Amelia co-chairs the center’s Pharmacy Quality Assessment Committee. She is an active member of the center’s Pharmacy & Therapeutics Committee, Chemotherapy Practice Committee, Ethics Committee, and the Joint Commission Medication Management Task Force. She is a member of the American Society of Health-System Pharmacists (ASHP) and the New York State Council of Health-system Pharmacists (NYSCHP).
Nina Cohen Orsini, Pharm.D., BCPS, BCIDP

PGY2 Infectious Diseases Residency Program Director
Nina Cohen Orsini is the PGY-2 Infectious Diseases residency program director (RPD) at Memorial Sloan Kettering Cancer Center. She received her doctor of pharmacy degree from the Arnold and Marie Schwartz College of Pharmacy and Health Sciences at Long Island University in Brooklyn, NY. Dr. Cohen Orsini completed a PGY1 Pharmacy Practice residency at Brookdale University Hospital in Brooklyn, NY and Infectious Diseases Pharmacy residency at The University of Rochester Medical Center, in Rochester NY. She has been working at MSKCC since 2005 as an Infectious Diseases Clinical Pharmacy Specialist. Dr. Cohen Orsini has many years of experience in infectious diseases pharmacotherapy and antibiotic stewardship, and has been a preceptor and mentor for residents, students and fellows. Dr. Cohen Orsini is board certified in pharmacotherapy with added qualifications in infectious diseases.
Clinical Pharmacy Specialist, Bone Marrow Transplant
Sonal Agarwal is a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplant (BMT) service at Memorial Sloan Kettering Cancer Center (MSKCC). Originally from Somerset, New Jersey, she earned her Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University in 2018. Dr. Agarwal completed an ASHP-accredited PGY1 Pharmacy Practice Residency and PGY2 Oncology Pharmacy Residency at Yale New Haven Health in New Haven, Connecticut. Following her training, she continued at Yale New Haven Health as a Clinical Pharmacy Specialist, providing care across both inpatient and outpatient hematology and oncology settings. She joined the Adult BMT team at MSKCC in 2023, where she currently serves as a preceptor for both inpatient and outpatient BMT pharmacy rotations.
Clinical Pharmacy Specialist, Cellular Therapy
Natalie Brumwell serves as a clinical pharmacy specialist with the Cellular Therapy Service at Memorial Sloan Kettering Cancer Center. She was born and raised in Lakeland, FL and received her Doctor of Pharmacy from the University of Florida College of Pharmacy in 2020. She went on to complete an ASHP accredited PGY1 Pharmacy Practice Residency and PGY2 Oncology Pharmacy Residency at Emory University Hospital in Atlanta, GA. Natalie started as a Cellular Therapy pharmacy specialist in 2022 and works in both the inpatient and outpatient setting. Her professional interests include precepting and mentorship, guideline development, patient education, and CAR T-cell and Tumor Infiltrating Lymphocyte (TIL) research.
Manpreet Boparai, PharmD, GCP, BCACP, BCGP
Clinical Pharmacy Specialist, Geriatrics
Manpreet Boparai serves as a Geriatric- Oncology Clinical Pharmacy Specialist at Memorial Sloan Kettering for over 10 years and has been serving as a residency preceptor since the initiation of the residency program at MSK. Born in Germany and raised in India and New York, Dr. Boparai, graduated from St. John’s University with a PharmD, and then went on to complete a specialized residency in geriatrics at Duke University and the DVAMC in North Carolina. She provides consultations to older adults in the Geriatrics Clinic for medication management. Dr. Boparai works with the interprofessional team to ensure that the medications prescribed for the patients contribute to the best possible health outcomes and to avoid, as possible, drug interactions and other medication-related adverse outcomes. She is committed to advancing healthcare literacy in her patients as well as in the community. She enjoys teaching and has held faculty positions at the university level. She has presented and taught courses locally and nationally about health and wellness for older adults. She is well published in journals and books about her work in medication management and polypharmacy in older adults with cancer.
Clinical Coordinator, Medication Reconciliation
Kristin Carotenuto is the Pharmacy Manager for the Medication Reconciliation program at Memorial Sloan Kettering Cancer Center. Kristin was born in New Jersey and received her Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers, The State University of New Jersey in 2003. After practicing as a pharmacy manager in community pharmacy for five years, Dr. Carotenuto joined MSK to launch the pharmacist driven medication reconciliation program. She is committed to optimizing patient care and serves on various hospital committees including The National Patient Safety Goals Task Force, Pharmacy QA, Pharmacy Operational Excellence Team, Reporting to Improve Safety and Quality Reading Group and Residency Advisory Committee for the PGY-2 Medication Use Safety & Policy program. Dr. Carotenuto is board certified in pharmacotherapy. She has trained many pharmacists in medication reconciliation and is a preceptor for the PGY-2 Medication Use Safety & Policy residency. Dr. Carotenuto is dedicated to serving the poor and marginalized and has participated in medical missions to South Africa and Madagascar where she lectured to pharmacists and oncologists.
Kristian Casem, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Kristian Casem serves as a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplant service at Memorial Sloan Kettering Cancer Center (MSKCC). He is originally from Edison, NJ and received his Doctor of Pharmacy from the Ernest Mario School of Pharmacy at Rutgers University in 2018. He then completed an ASHP accredited PGY-1 Pharmacy Practice Residency at Saint Peter’s University Hospital in New Brunswick, NJ and an ASHP accredited PGY-2 Oncology Pharmacy Residency at the University of Pittsburgh Medical Center (UPMC) Presbyterian-Shadyside Hospital. After completing residency, he worked as a clinical pharmacy specialist at Emory Healthcare in Atlanta, GA, practicing in sarcoma, breast cancer, and genitourinary cancers. He joined MSK in 2023 and precepts for the adult BMT rotation.
Clinical Pharmacy Specialist, Bone Marrow Transplant
Melinda Cook serves as a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplant service at Memorial Sloan Kettering Cancer Center (MSKCC). She is originally from St. Petersburg, Florida and received an undergraduate from the Florida State University in 2016. She then went on to complete her Doctor of Pharmacy in 2020 from the University of Michigan College of Pharmacy. Following pharmacy school, Dr. Cook completed an ASHP accredited PGY1 Pharmacy Practice Residency at Novant Health Presbyterian Medical Center in 2021. She then went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency at Dana-Farber Cancer Institute in 2022. She began her career at MSK in 2022 as a BMT clinical pharmacy specialist and looks forward to precepting both the inpatient and outpatient adult BMT rotations.

Ryan Daley serves as a Preceptor for the PGY2 Oncology Residency Program (Adult-Focused) at Memorial Sloan Kettering Cancer Center. He was born in Syracuse, NY, and received his Doctor of Pharmacy degree from The State University of New York at Buffalo School of Pharmacy and Pharmaceutical Sciences. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Fletcher Allen Health Care in Burlington, VT, and went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency at The University of Texas MD Anderson Cancer Center. Dr. Daley began his clinical career at Fletcher Allen Healthcare in 2004 as a clinical pharmacy specialist in oncology where he precepted PGY1 residents and pharmacy students. In 2013 Dr. Daley began his career at Memorial Sloan Kettering Cancer Center as a Clinical Pharmacy Specialist in Adult Lymphoma & Leukemia, eventually focusing solely on Leukemia. Dr. Daley is board certified in oncology pharmacy, is a member of the MSK Pharmacy Protocol Review Committee, and is a participating member of the Pharmacy Committee of the national cooperative investigational group, The Alliance for Clinical Trials in Oncology.

Nicole Daukshus serves as a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center. She is from New Jersey and received her pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University in 2017. After pharmacy school, she went on to complete her PGY1 residency at Hackensack University Medical Center in New Jersey and subsequently went on to complete her PGY2 Pediatric Oncology Residency at MSK.

Jacqueline Dela Peña serves as a Clinical Pharmacy Specialist with the Cellular Therapy Service at Memorial Sloan Kettering Cancer Center. She earned her Bachelor of Science in Neuroscience from the University of Michigan in 2016 and her Doctor of Pharmacy from the University of Michigan College of Pharmacy in 2020. Dr. Dela Peña completed an ASHP-accredited PGY1 Pharmacy Practice Residency at Michigan Medicine in Ann Arbor, MI followed by an ASHP-accredited PGY2 Hematology/Oncology Pharmacy Residency at the University of North Carolina Medical Center in Chapel Hill, NC. She began her career at MSKCC in 2022 as a Clinical Pharmacy Specialist in Adult Leukemia prior to her transition to the Cellular Therapy Service in 2024. Her professional interests include CAR-T cell toxicity management and cell therapy in solid tumors.

Raymond DeMatteo serves as a Clinical Pharmacy Specialist for the Early Drug Development Department at Memorial Sloan Kettering Cancer Center (MSK). He is originally from upstate New York and received his Doctor of Pharmacy from LIU Brooklyn in 2018. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Northwestern Memorial Hospital in Chicago, IL, followed by a PGY2 Oncology Pharmacy Specialty Residency at MSK. Dr. DeMatteo started as a lymphoma specialist in 2020 before transitioning to the Early Drug Development Department in 2022. His academic interests include precision medicine, immunotherapeutics, and teaching.

Stephen Eng serves as a Clinical Pharmacy Specialist for the Adult Bone Marrow Transplantation Program at Memorial Sloan Kettering Cancer Center (MSK). He is from Queens, NY and began his pharmacy career at MSK in 2014 as part of the pharmacy student intern program. He received his Doctor of Pharmacy degree in 2018 from St. John’s University in Queens, NY. Following pharmacy school, Dr. Eng completed an ASHP accredited PGY1 Pharmacy Residency at Hackensack University Medical Center in Hackensack, NJ followed by a PGY2 Adult Oncology Pharmacy Residency here at MSK. In 2020, Dr. Eng began his career at MSK as a clinical pharmacy specialist in bone marrow transplantation and is currently serving as the Residency Program Coordinator. He is a Past President of the New York City Society of Health-system Pharmacists and serves on the State Council’s Oncology Committee. His professional and clinical interests include mentoring, teaching, and conducting research on graft-versus-host disease.
Mariel Flanagan, PharmD
Clinical Pharmacy Specialist, Pediatric Transplant and Cellular Therapy
Mariel Flanagan is a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from St. Petersburg, Florida and received an undergraduate degree from Lehigh University in 2017. She then went on to complete her Doctor of Pharmacy from the Creighton University School of Pharmacy and Health Professions in 2022. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at Ann & Robert H. Lurie Children’s Hospital of Chicago, then went on to complete a PGY2 Pediatric Oncology Residency at MSK. Dr. Flanagan began her career as a pediatric bone marrow transplant pharmacy specialist in August of 2024 and currently rotates between inpatient and outpatient services.
Kate Gasparini, PharmD, BCOP, BCPPS
Clinical Pharmacy Specialist III, Pediatric Hematology/Oncology
Kate Gasparini is a Pediatric Oncology Clinical Pharmacy Specialist at Memorial Sloan Kettering Cancer Center (MSK). She was born and raised in New Jersey and received her Doctor of Pharmacy from the University of Rhode Island College of Pharmacy in 2013. Following pharmacy school, she completed an ASHP accredited PGY1 Pharmacy Practice Residency (2014) and PGY2 Pediatric Pharmacy Specialty Residency (2015) at Children’s Hospital Colorado in Aurora, CO. Dr. Gasparini began her career as a clinical pharmacy specialist in pediatric oncology at Texas Children’s Hospital in Houston, TX where she served as the liaison for the solid tumors and neuro-oncology teams. In 2018, Dr. Gasparini joined the pediatric clinical pharmacy team at MSK, and her clinical practice includes rotating between the inpatient oncology, outpatient hematologic malignancy, outpatient neuro-oncology, and outpatient neuroblastoma clinical services. Currently, Dr. Gasparini serves as a clinical preceptor for PGY2 oncology residents and pharmacy students in these areas, as well as a research mentor for PGY2 pediatric oncology resident research projects. She is an active member of the Hematology/Oncology Pharmacy Association (HOPA), Pediatric Pharmacy Association (PPA), and Children’s Oncology Group (COG) organizations. Dr. Gasparini is dual board certified in oncology and pediatric pharmacy.
Tami Guitaud, PharmD
Manager, Main Campus Chemotherapy Pharmacy Operations
Tami Guitaud is the Main Campus Chemotherapy Pharmacy Manager at Memorial Sloan Kettering Cancer Center (MSK). Although born and raised in NYC, she attended The Ohio State University and obtained both her B.S. in Pharmacy ’01 and Doctor of Pharmacy Degree ’06. Dr. Guitaud began her career as a hospital pharmacist, which included chemotherapy order verification, preparation and dispensing of hazardous drugs. After receiving her PharmD, she began practicing as an Adult Chemotherapy Pharmacist and was later promoted into the Investigational Oncology Pharmacist role.
In 2013, Dr. Guitaud joined MSK as a pediatric oncology pharmacist, and in 2019 she was promoted into the Pediatric Oncology Clinical Coordinator role, where she served on multidisciplinary committees that worked collaboratively to provide quality care for their patients. In 2021, Dr. Guitaud transitioned into her current role, where she manages the chemotherapy pharmacy operations for admitted patients at main campus, and the outpatient pediatric chemotherapy services at their Pediatric Ambulatory Care Center and the MSK regional sites. She serves as a mentor for SNPhA pharmacy students and is a preceptor for the PGY2 Pediatric Oncology Pharmacy Residents at MSK.
Clinical Pharmacy Specialist, Multiple Myeloma Service
Dr. Issam Hamadeh is a Clinical Pharmacy Specialist for the multiple myeloma service at Memorial Sloan Kettering Cancer Center. He received his Doctor of Pharmacy degree from Purdue University, IN. After graduation, he completed a 3-year fellowship in Precision Medicine/Pharmacogenetics at the University of Florida, FL. Dr. Hamadeh is board certified in Oncology, and a member of several professional organizations. His clinical interests include supportive care, implementation of pharmacogenetics in hematology/oncology, and research.
Stephen Harnicar, PharmD, BCOP
Manager, Clinical Pharmacy Services & Research
Stephen Harnicar is a Clinical Pharmacy Manager at Memorial Sloan Kettering Cancer Center (MSK). He received his PharmD degree in 2006 from Duquesne University in Pittsburgh, PA. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at The Johns Hopkins Hospital in Baltimore, MD and continued to an ASHP accredited PGY2 Oncology Pharmacy Residency at MSK. Post residency, Dr. Harnicar joined MSK as a clinical pharmacy specialist practicing in leukemia and bone marrow transplant which included responsibilities as a preceptor for both the PGY2 adult oncology residents and pharmacy students. In 2014, Dr. Harnicar took on a role as clinical manager for the Department of Pharmacy with a focus on the management of the Pharmacy Department’s clinical research goals. Dr. Harnicar currently serves on the IRB as well as having an active role in departmental QA initiatives. His clinical interests include hematologic malignancies, pharmacokinetic, and pharmacogenomic research; and has spoken internationally on these topics.
Lynnette Henshaw, PharmD, BCOP
Clinical Pharmacy Specialist, Lymphoma
Lynnette Henshaw serves as a Clinical Pharmacy Specialist for the lymphoma team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from New Jersey and received her Doctor of Pharmacy degree in 2019 from St. John’s University in Queens, NY. Dr. Henshaw completed an ASHP accredited PGY1 Pharmacy Practice Residency at New York-Presbyterian Brooklyn Methodist Hospital followed by a PGY2 Pharmacy Oncology Residency at Boston Medical Center. After completing residency, she practiced as a clinical pharmacy specialist for Boston Medical Center as a clinical pharmacy specialist for inpatient & outpatient hematology and oncology. She joined the MSK team in 2023 and looks forward to serving as a preceptor for the inpatient and outpatient lymphoma rotations. Dr. Henshaw is board certified in Oncology and her professional interests include precision medicine, patient education, and precepting/mentorship.
Sean Hwang, PharmD
Clinical Pharmacy Specialist, Leukemia
Sean Hwang serves as a Clinical Pharmacy Specialist in the Leukemia Department at Memorial Sloan Kettering Cancer Center (MSK). He is originally from Seattle, Washington and earned his Doctor of Pharmacy degree from the University of Washington School of Pharmacy. He completed both his ASHP-accredited PGY1 Pharmacy Practice Residency and PGY2 Oncology Pharmacy Residency at the University of Washington Medical Center/Fred Hutchinson Cancer Center in Seattle, WA. Following his PGY2 training, Dr. Hwang joined MSK in 2024 as a Clinical Pharmacy Specialist in Leukemia. His professional interests include acute myeloid leukemia, immunotherapy, and mentoring students/residents.
Adam Kahleifeh, PharmD
Clinical Pharmacy Specialist, Neuro-Oncology
Adam Kahleifeh serves as a clinical pharmacy specialist with the Neuro-Oncology service at Memorial Sloan Kettering Cancer Center (MSK). He grew up in Kentucky and received his Doctorate of Pharmacy from the University of Kentucky College of Pharmacy in 2019. He completed his ASHP accredited PGY1 at Baptist Health in Louisville, Kentucky. Dr. Kahleifeh then went on to complete his ASHP accredited PGY2 Adult Oncology Residency at MSK. He currently rotates between both inpatient and outpatient services. His professional interests include teaching, mentorship, and clinical research.

Jung is a Clinical Pharmacy Specialist in Infectious Diseases at Memorial Sloan Kettering Cancer Center. He was born in Boston, MA and grew up in Korea. He received his Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy at Rutgers University in 2020. He then completed a PGY-1 Pharmacy residency at SUNY Downstate Health Sciences University in Brooklyn, NY, and a PGY-2 Infectious Diseases Pharmacy residency at Memorial Sloan Kettering Cancer Center. Clinical Pharmacy Specialist, Pediatric Hematology/Oncology
Varshini Kanukollu serves as a Pediatric Oncology Clinical Pharmacy Specialist at Memorial Sloan Kettering Cancer Center (MSK). She is originally from New Jersey and received her Doctor of Pharmacy degree in 2021 from Rutgers University Ernest Mario School of Pharmacy. Dr. Kanukollu completed an ASHP-accredited PGY1 Pharmacy Practice Residency at Hackensack University Medical Center followed by the completion of a PGY2 Pediatric Oncology Pharmacy Residency at MSK in 2023. After completing residency, she practiced as a Pediatric Hematology/Oncology and Bone Marrow Transplant Clinical Pharmacy Specialist at Northwell Health Cohen Children’s Medical Center and returned to MSK in 2025. Dr. Kanukollu is board-certified in oncology and her professional interests include precepting, supportive care, survivorship, and clinical research in the pediatric oncology setting.
Lauren Koranteng, PharmD, BCPS, CPE
Clinical Pharmacy Specialist II, Supportive Care Service
Lauren Koranteng is a Clinical Pharmacy Specialist in the Department of Pharmacy at the Memorial Sloan Kettering Cancer Center (MSK) in Manhattan, New York. Since 2013, she has served as a member of the interdisciplinary team on the Supportive Care Service, contributing pharmacotherapy expertise both inpatient and outpatient. She obtained her undergraduate degree from Mount Holyoke College in Neuroscience and Behavior with a minor in Complex Organizations. She earned her Doctor of Pharmacy Degree from Temple University followed by an American Society of Health Systems Pharmacists (ASHP) accredited PGY1 residency at the University at Buffalo/Niagara Hospice and an ASHP accredited PGY2 Pain Management and Palliative Care Residency at Johns Hopkins Hospital. Dr. Koranteng serves as a clinical preceptor to MSK pharmacy residents for the Pain Management and Palliative Care rotation. She is board-certified in pharmacotherapy and has membership in several professional organizations. Dr. Koranteng’s special interests include pain management and access to safe oncology and supportive care medicines in developing countries.

Binni Kunvarjee is a clinical pharmacy specialist with the pediatric bone marrow transplant team at Memorial Sloan Kettering Cancer Center (MSK). She is originally from the northern New Jersey area and received her Doctorate of Pharmacy from the Ernest Mario School of Pharmacy at Rutgers University in 2015. She completed her ASHP accredited PGY-1 at Hackensack University Medical Center in New Jersey, then went on to complete her PGY2 Pediatric Oncology Residency at MSK. Dr. Kunvarjee began her career as pediatric bone marrow transplant pharmacy specialist at MSK in August 2017 and currently rotates between inpatient and outpatient services. Edward Lee serves as a Clinical Pharmacy Specialist for the Neuro-Oncology department at Memorial Sloan Kettering Cancer Center (MSK). He is originally from Philadelphia and received his Doctor of Pharmacy from the University of North Carolina in 2020. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Emory University Hospital in Atlanta, GA followed by a PGY2 Adult Oncology Pharmacy Specialty Residency at Memorial Sloan Kettering Cancer Center in 2022. His clinical interests include teaching and precepting, clinical research, and mentorship.
Director, Chemotherapy Practice and Investigational Drug Service
Melissa Lee-Teh is the Director for Chemotherapy Practice and Investigational Drug Service at Memorial Sloan Kettering Cancer Center (MSK). She received her BS in Pharmacy from Arnold & Marie Schwartz College of Pharmacy at Long Island University and her Doctor of Pharmacy degree from University of Kansas. Dr. Lee-Teh has over 20 years of pharmacy experience in the health care system and currently oversees pharmacy chemotherapy operations across MSK’s inpatient and outpatient areas. She also serves as an investigator on several MSK research protocols and leads pharmacy investigational drug services for both adult and pediatric services. She is an active member on numerous MSK committees, including Chemotherapy Practice, Pharmacy & Therapeutics, Joint Commission Medication Management, and Pharmacy Quality Assessment.
Andrew Lin, PharmD, BCOP
Clinical Pharmacy Manager, Bone Marrow Transplant
Andrew Lin is the Clinical Pharmacy Manager for the Adult BMT and Cellular Therapy Services. He received his Doctor of Pharmacy degree in 2009 from the University of North Carolina Eshelman School of Pharmacy at Chapel Hill. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at Carolinas Medical Center in Charlotte, NC. Dr. Lin continued his training by completing an ASHP accredited PGY2 Oncology Pharmacy Residency at the University of Washington Medical Center/Seattle Cancer Care Alliance. He began his career at MSK in 2011 as a clinical pharmacy specialist initially with the lymphoma and leukemia services before transitioning to the bone marrow transplant service. In 2024 he transitioned into his current role as a manager within the Department of Pharmacy. Dr. Lin is active in clinical research and in 2024 he was awarded the ASTCT Pharmacy SIG Clinical Excellence Award. He has also presented at several national meetings including ASHP, HOPA, ATOPP, and the Tandem meetings of ASTCT and CIBMTR.
Stephanie Monaco-Cermak, PharmD, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Stephanie Monaco-Cermak is a BCOP certified clinical pharmacy specialist in bone marrow transplant. Stephanie grew up in NY but has moved around the country, earning her BS in Chemistry from Florida Institute of technology, and then her PharmD from the University of Utah. She completed her PGY1 residency at the Salt Lake City Veterans Hospital, then completed a PGY2 Adult Oncology residency at New York-Presbyterian. She joined MSK in 2020 and currently rotates between both inpatient and outpatient services.
Mary Nauffal, PharmD, MS, BCOP
Clinical Pharmacy Specialist, Bone Marrow Transplant
Mary Nauffal is a clinical pharmacy specialist on the adult BMT service who is board certified in oncology pharmacy. She was born in Sydney, Australia then moved and grew up in Lebanon and later moved to Boston in 2013. She received her Doctor of Pharmacy from the Lebanese American University and a Master of Science in Pharmacology from Northeastern University. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at the Brigham and Women’s Hospital and her PGY2 Oncology Pharmacy Residency at the Dana-Farber Cancer Center, both in Boston. She began her career at Brigham and Women’s/Dana-Farber Cancer Center as a clinical pharmacy specialist initially with the leukemia service before transitioning to the bone marrow transplant service. She joined MSK in 2020 as a bone marrow transplant pharmacy specialist and has been a preceptor and mentor for residents.
Lane Nguyen, PharmD, BCPPS
Clinical Pharmacy Specialist, Pediatric Critical Care
Lane Nguyen is the Pediatric Critical Care Clinical Pharmacy Specialist at Memorial Sloan Kettering Cancer Center. Dr. Nguyen received his Doctor of Pharmacy degree from The University of Iowa College of Pharmacy in 2016. He completed an ASHP accredited PGY1 Pharmacy Practice Residency at The University of Iowa Hospitals and Clinics. Dr. Nguyen then went on to complete an ASHP accredited PGY2 Pediatric Pharmacy Residency at The University of Iowa Stead Family Children’s Hospital. In 2018, Dr. Nguyen joined Memorial Sloan Kettering Cancer Center as a member of the pediatric clinical pharmacy team. Clinical interests include cardiopulmonary resuscitation, analgosedation, and mentoring students. Additionally, he serves as an active member of the Society of Critical Care Medicine (SCCM).
Jasmine Patel, PharmD, BCOP
Clinical Pharmacy Specialist, Thoracic Medical Oncology
Jasmine Patel serves as a Clinical Pharmacy Specialist for the Thoracic Oncology department at Memorial Sloan Kettering Cancer Center (MSK). She is originally from New Jersey and received her Doctor of Pharmacy from the Ernest Mario School of Pharmacy at Rutgers University in 2018. Dr. Patel completed an ASHP-accredited PGY-1 Pharmacy Practice Residency at Hartford Hospital in Hartford, Connecticut followed by a PGY-2 Oncology Residency at Boston Medical Center (BMC) in Boston, MA. After completing residency, she worked at BMC as a Clinical Pharmacy Specialist in hematology/oncology focusing on oral oncolytics, and then transitioned to Clinical Pharmacy Coordinator for hematology/oncology overseeing the ambulatory clinics and specialty pharmacy, and the PGY-2 Oncology Residency Program. During her time there, she developed quality improvement (QI) initiatives focused on minimizing disparities of care for cancer patients, was awarded $600,000 in educational and QI grants, and lectured at the Boston University School of Medicine. She is actively involved with HOPA, NCODA, and HOPCON. Dr. Patel’s clinical interests include biomarker-driven therapy, disparities in cancer care, and teaching.
Rebecca Razel, PharmD, BCPS
Clinical Coordinator of Quality Assessment and Improvement
Rebecca Razel is the Clinical Coordinator of Quality Assessment and Improvement at Memorial Sloan Kettering Cancer Center. Dr. Razel is originally from Western New York and received her Doctor of Pharmacy degree from the State University of New York at Buffalo in 2007. She completed a PGY-1 Pharmacy Practice Residency within the One Brooklyn Health System and maintains board certification in pharmacotherapy. She then spent over a decade practicing as an inpatient pharmacist, joining MSK’s pharmacy team in 2019. Dr. Razel transitioned to her quality and safety role in 2023 with the goal of applying her hands-on experience to process improvement and medication error reduction. She looks forward to working with MSK’s pharmacy residents on medication event analysis.

Richard Shin serves as a Clinical Pharmacy Specialist for the Leukemia service at Memorial Sloan Kettering Cancer Center (MSKCC). He is originally from Chicago, IL and received his Doctor of Pharmacy from University of Illinois Chicago in 2022. He then completed an ASHP accredited PGY-1 Acute Care Pharmacy Practice Residency and an ASHP accredited PGY-2 Oncology Pharmacy Residency at Stanford Health Care in Stanford, CA. After completing residency, he joined MSK in 2024, and he currently rotates between both inpatient and outpatient services and precepts for the Leukemia rotation.

Krisoula Horiates Spatz serves as a pediatric oncology clinical pharmacy specialist at Memorial Sloan Kettering Cancer Center (MSK). She was born in Texas, raised in New Jersey, and received her Doctor of Pharmacy degree in 2013 from the Ernest Mario School of Pharmacy of Rutgers University. She completed an ASHP accredited PGY1 Pharmacy Practice Residency at the Penn State Milton S. Hershey Medical Center in Hershey, PA. Dr. Spatz then went on to complete an ASHP accredited PGY2 Oncology Pharmacy Residency (Pediatric-Focused) at MSK. Dr. Spatz began her career at MSK in 2015 as a clinical pharmacy specialist in pediatrics. She also serves as a clinical preceptor to PGY2 oncology residents as well as pharmacy students in the areas of inpatient pediatric oncology and outpatient hematologic malignancies and neuro-oncology. Dr. Spatz is board certified in oncology pharmacy and is an ASHP Well-Being Ambassador, having completed the Well-Being and Resilience Certificate.

Erica Tonti serves as a pediatric oncology clinical pharmacy specialist at Memorial Sloan Kettering Cancer Center (MSK). She was born and raised in Brooklyn, NY and received her Doctor of Pharmacy degree in 2022 from St. John’s University in Queens, NY. She completed an ASHP accredited PGY1 Pharmacy Practice Residency in 2023 and subsequently completed a PGY2 Pediatric Pharmacy Specialty Residency in 2024 at Children’s Hospital Colorado in Aurora, CO. Dr. Tonti began her career as a clinical pharmacy specialist in pediatrics at MSK in 2024 and currently rotates between inpatient oncology, outpatient hematologic malignancy, outpatient neuroblastoma, and outpatient neuro-oncology clinical services.
Katherine S. Vicinanza-Rickard, PharmD, BCPS
Clinical Coordinator, Drug Policy Management
Katherine Vicinanza-Rickard is a Drug Policy preceptor for the PGY2 Medication-Use Safety & Policy Residency Program at Memorial Sloan Kettering Cancer Center. She also serves as a preceptor for the PGY2 Pediatric Oncology & Infectious Disease Residency Programs for a rotation in Medication Event Analysis, Management and Error Prevention Strategies Dr. Vicinanza-Rickard is originally from Queens, New York, and received her Doctor of Pharmacy degree from MCPHS University-Boston Campus. She completed her PGY-1 Pharmacy Residency at the Northport Veterans Affairs Medical Center, and then went on to complete a PGY-2 Pharmacy Residency in Medication-use Safety at Memorial Sloan Kettering Cancer Center. Dr. Vicinanza-Rickard is currently a Clinical Coordinator in Drug Policy Management where she implements revisions in the formulary database, participates in RISQ events analysis, provides drug information to pharmacists and clinicians, updates and manages parenteral guidelines, and contributes to projects related to clinical, quality, safety, and financial outcomes of drug use. Dr. Vicinanza-Rickard is board certified in pharmacotherapy.
Clinical Pharmacy Specialist, Lymphoma
Alice Wang is a Clinical Pharmacy Specialist on the multiple myeloma team at Memorial Sloan Kettering Cancer Center. She received her Doctor of Pharmacy degree from Purdue University College of Pharmacy in 2020. She then completed an ASHP accredited PGY-1 Pharmacy Residency at Maine Medical Center in 2021 and PGY-2 Oncology Pharmacy Residency at MSK in 2022. After residency, she joined the multiple myeloma team and precepts the inpatient and outpatient multiple myeloma rotations. Her professional interests include precepting, guideline development, and patient advocacy.
Manager, BAIC/Kimmel Pharmacy Operations
Stacy Wong serves as the preceptor of the pharmacy operations rotation in the Medication Safety Residency at Memorial Sloan Kettering Cancer Center. She is also a member of the Residency Advisory Committee. Dr. Wong grew up in Delaware and received her Doctor of Pharmacy from Rutgers, the State University of New Jersey in 2008. She decided to try the city life and moved to the Big Apple after graduating to start her career at Memorial Sloan Kettering Cancer Center. She has staffed in various areas including inpatient areas, pediatric outpatient chemotherapy, and adult outpatient clinics. In January 2018, Stacy received her Master of Science in Health Sciences in Regulatory Affairs from the George Washington University.
